dc.description.abstractThis report is one among four that represents an evaluation of Ohio s Assisted Living Medicaid Waiver program. This part assessed how well the program performed in developing and delivering services. Specifically, the evaluation addressed the effectiveness of the service plan process in preserving the consumers independence, privacy, and choice. It also looked at the factors that lead to service tier change in the first 90 days after enrollment and those that result in a permanent tier re-assignment.en_US
