Abstract
There is a growing interest in Medicaid managed long-term services and recent expansion legislation now allows Medicare Advantage plans to incorporate an array of community-based services into their benefit packages. This study was a first step in gaining a better understanding of the care management plan models used in the Ohio MyCare Demonstration, focusing on the Aetna unified care management model implemented in three regions of the state in partnership with three area agencies on aging (AAAs). The report reviews the characteristics of Aetna MyCare participants, describes Aetna’s care management model, and identifies challenges, strengths, and areas for improvement.
