dc.description.abstractThe Aging and Disability Business Institute (Business Institute), led by the National Association for Area Agencies on Aging (n4a), was established in 2016 with a mission to build and strengthen partnerships between aging and disability community-based organizations and the health care system. The goal of the Business Institute was to increase contracting between CBOs and health care entities, thereby addressing social and behavioral determinants of health through integrated care. This report presents findings from an evaluation of Business Institute activities and outputs through December 2018.en_US
